The Awards Team of the Sports Physical Therapy Section is soliciting nominations for the following awards for the year 2011.

Outstanding Student Award

New Horizon Award

Excellence in Education Award

Lifetime Excellence in Education Award

Lynn Wallace Clinical Education and Professional Development Award

Outstanding Service Award

Presidential Award of Merit

Jack C. Hughston Sports Physician Award

Ron Peyton Award Turner A. Blackburn Hall of Fame/Lifetime Achievement Award

Procedure for Review and Selection:

1. Nomination materials must include an original and four copies submitted to the Awards Team on or before September 15, 2010. Submissions received after September 15, 2010 will not be considered.
2. The Awards Team will review nominations and recommend the most qualified candidate to the Executive Committee.
3. The award will be presented only if there are qualified candidates and one is selected. Nomination materials are considered property of the Awards Team, who will maintain their confidentiality.
4. Nomination materials will be returned. If any individual is not selected for the award in a given year, that individual may be nominated in subsequent years by a resubmission of updated documents.
5. All nominators are requested to confirm that their nominee is available to attend CSM before submitting the nomination.
